Welcome to Pleasant Valley Health Services (“PVHS,” “we,” “our,” or “us”). These Terms of Service (“Terms”) govern your access to and use of our website, online services, telehealth platforms, and in-person healthcare services (collectively, the “Services”).

By accessing or using our Services, you agree to these Terms. If you do not agree, please do not use our Services.

Eligibility

You must be:

  • At least 18 years old to create an account or receive Services on your own behalf.
  • A parent, guardian, or legal representative to request Services for a minor.
  • Located in a state (MD, DC, or VA) where PVHS providers are licensed to practice.

The information provided on our website (including blog posts, educational resources, and social media content) is for general informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment.

Patient-Provider Relationship

  

A patient-provider relationship with PVHS is only established when:

  1. You have completed our new patient registration process;
  2. You have signed all required consent forms; and
  3. You have had an initial consultation with a PVHS licensed provider.

Simply using our website or sending an inquiry does not establish a patient-provider relationship.

Telehealth Services

  

 When using our telehealth services:

  • You consent to receiving care via a secure, HIPAA-compliant video or audio platform.
  • You understand that telehealth has limitations, and in some cases, in-person evaluation may be required.
  • You agree to provide accurate location information during each telehealth session, as state licensure  laws apply.

Privacy & HIPAA Compliance

  Your personal health information is protected by our Privacy Practices Policy and by applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

User Responsibilities

  You agree to:

  • Provide accurate, current, and complete information when registering for Services.
  • Keep your account login credentials secure and confidential.
  • Use our Services only for lawful purposes.
  • Not transmit any malicious code, spam, or unauthorized advertisements through our systems.

Payment & Insurance

  

  • Fees for services are due at the  time of service unless prior arrangements are made.
  • If we accept your insurance, you remain responsible for any co-pays, deductibles, or non-covered services.
  • We may update our accepted insurance plans and fee schedules at any time.

Cancellations & No-Shows

  

  • Fees for services are due at the  time of service unless prior arrangements are made.
  • If we accept your insurance, you  remain responsible for any co-pays, deductibles, or non-covered services.
  • We may update our accepted insurance plans and fee schedules at any time.

Cancellations & No-Shows

  

  • We require at least 24 hours’ notice to cancel or reschedule an appointment.
  • Missed appointments or late cancellations may result in a fee as outlined in our clinic policy.

No Warranty

  

While we strive to provide accurate and up-to-date information, PVHS does not guarantee that our Services or website will be error-free, uninterrupted, or always available.

Limitation of Liability

  

To the fullest extent permitted by law, PVHS, its providers, and staff are not liable for any damages resulting from:

  • Use or inability to use the Services.
  • Unauthorized access to your information; or
  • Reliance on any content obtained through the website outside of a formal patient-provider relationship.
