  • Located in a state (MD, DC, or VA) where PVHS providers are licensed to practice.

A patient-provider relationship with PVHS is only established when:

  1. You have completed our new patient registration process;
  2. You have signed all required consent forms; and
  3. You have had an initial consultation with a PVHS licensed provider.

Simply using our website or sending an inquiry does not establish a patient-provider relationship.

Telehealth Services

  

 When using our telehealth services:

  • You consent to receiving care via a secure, HIPAA-compliant video or audio platform.
  • You understand that telehealth has limitations, and in some cases, in-person evaluation may be required.
  • You agree to provide accurate location information during each telehealth session, as state licensure  laws apply.

Privacy & HIPAA Compliance

Cancellations & No-Shows

  

  • Fees for services are due at the  time of service unless prior arrangements are made.
  • If we accept your insurance, you  remain responsible for any co-pays, deductibles, or non-covered services.
  • We may update our accepted insurance plans and fee schedules at any time.

Cancellations & No-Shows

  

  • We require at least 24 hours’ notice to cancel or reschedule an appointment.
  • Missed appointments or late cancellations may result in a fee as outlined in our clinic policy.

While we strive to provide accurate and up-to-date information, PVHS does not guarantee that our Services or website will be error-free, uninterrupted, or always available.
