FMLA, Disability & Administrative Documentation Policy

FMLA, Disability, Leave of Absence, and Administrative Paperwork Policy

 To ensure accurate clinical assessment and documentation, our practice completes FMLA, disability, workplace accommodation, return-to-work, school accommodation, and similar administrative forms only for established patients actively receiving treatment within the practice.

Eligibility

  • Patients must be established with the practice and have an ongoing treatment relationship with the provider.
  • Requests from new patients, one-time consultations, or individuals seeking documentation solely for FMLA, disability, or leave purposes will not be completed.
  • Documentation must be supported by sufficient clinical history, treatment records, and evidence of functional impairment.

Important Information

  1. Completion of forms is not guaranteed and is based solely on clinical findings, medical necessity, and the provider's professional judgment.
  2. A dedicated appointment may be required to review symptoms, functional limitations, treatment history, and the requested documentation.
  3. Patients must complete all patient sections of forms before submission to the practice.
  4. Forms will not be completed during routine medication management appointments unless sufficient time has been specifically scheduled.
  5. Additional documentation, letters, narrative reports, or appeals may require separate appointments and administrative fees.

Processing Time

Please allow up to 10 business days for completion of paperwork after all required forms and supporting information have been received. Expedited requests cannot be guaranteed.

Clinical Determination

The provider's role is to document clinical findings and functional limitations supported by the medical record. Approval or denial of leave, disability benefits, accommodations, or workplace requests is determined by the employer, school, insurance carrier, or other reviewing entity.

Ongoing Care Requirement

Patients receiving FMLA, disability, or accommodation documentation may be required to maintain recommended follow-up appointments and treatment. Failure to remain engaged in treatment may affect the provider's ability to complete future documentation requests.

By submitting paperwork for completion, the patient acknowledges understanding of this policy and agrees to the requirements outlined above.
